It used to fail every time once the vault had locked itself, and failed the same way whether the password was right or wrong, so the only way back in was to open the vault page. A refusal now says what it was (wrong password, wrong code, a code is needed, or too many attempts) and, when the vault is making you wait, how long that wait is.
