Saved passwords no longer go missing when the vault is busy

Importing a password file while saving a login, or two saves landing at once, could leave the vault holding a document neither one wrote, and "delete my vault" could be silently undone by a save made moments earlier. Every change to the vault now happens one at a time, and the app only shows a change as saved once it is really on disk, so a failed write says so instead of pretending.
