Reporting an incident asks for the date and time it occurred (defaulting to now), the site area, a linked inspection, witnesses and injury details, and offers a notifiable-incident toggle that turns itself on for high or critical severity or when injuries are recorded, with guidance and a running clock showing how long ago it happened. All of it is editable afterwards, shows on the incident, and is included in the CSV export.
