When an external app registered a workspace through our API, the access it was meant to grant could silently fail, leaving people unable to open the workspace they had just been added to. Those grants now go through correctly, repeated grants no longer pile up duplicate access records, and the API reports a clear error instead of a false success if anything goes wrong.
